• Hot lunch begins Monday, August 26. Bring cold lunch and beverage (healthy, please) August 22 and 23.
• Medication Consent - Physician
• Medical Provider Authorization Form - Prescription Medication (Epi-Pen & Inhaler)
• Non-Prescription Medication Consent
• Any medication sent to school (including but not limited to cough drops, Tylenol, Benedryl, prescription meds, etc) must have a medical consent form attached to the medication

• Safe Environment Education
• Anyone who is volunteering in any manner at St. Anthony on the Lake needs to participate in a one-time Safe Environment Education session. Held at St. Anthony on the Lake from 6 - 9 p.m. on August 28, 2024. Other sessions available at archmil.org.
